Please fill out each field completely. All vacation requests must be submitted at least two weeks before your desired dates. 

In the event of an absence, we will review your request to determine if any administrative or financial adjustments are necessary. All requests are subject to the City Cathedral Church Calendar and require administrative approval.

Failure to complete this request can result in a forfeiture of any financial arrangements and disciplinary action.
